Card Access Request Form

In order to be approved for all-hours access, you must complete some laboratory safety training:

a. Attach a certificate of completion or a confirmation that you have completed a general lab safety course within the last year. If you have not completed such a course, you can find a course labeled Chemical Hygiene – EHS at utah.bridgeapp.com.

b. Complete a walkthrough safety tour of the Crus Center to learn the safety protocols and instructions specific to the space. The lab manager or staff will reach out to you once we receive this form to schedule that walkthrough.
